Seniors and Physical Disabilities
Elderly persons aged 65 and older as well as physically disabled adults and children served are those individuals who are experiencing difficulty living independently or with family and are unable to provide for their own needs. The major purpose is to offer a continuum of community-based services to allow for achievement or maintenance of independent living and self-sufficiency. Much of this is accomplished through contracts with several home care providers in the county and elsewhere to enlist their medically skills staff and resources as support to persons assessed at risk for skilled nursing facility or others institutional based care.
