These publicly funding long-term programs support people with a variety of disabilities, including: developmental disabilities, mental illness, chronic medical conditions, acquired or traumatic brain injuries and physical disabilities. When possible, we partner with personal care and support agencies to provide flexible, cost-effective home and community-based services.
