Enclose in your envelope:

• Motor Vehicle/DNR Registration Renewal
• A check or money order. Your check must include Driver License number, phone number and correct address.
• Insurance company's name, the policy number and the policy expiration date for the motor vehicle or motorcycle. Insurance information is not required for trailers.
